
Adult Day Services: Our adult day services provides individual goals and programs to work on for those with any form of disability. We offer structured exercise programs, socialization activities, cultivation of personal interests, activities such as group projects and recreational activities, health services, ROM, education on good health practices and medication administration. Our adult day individuals attend outcome based classes weekly and attend a community outing every Thursday. The adult day group focuses on learning the skills needed to move into the work force and have all the skills that are required. Staffing for the adult day service is usually either at a 1:4 or 1:1 ratio, depending on the client specific needs.
Youth Transitional Services: Transitional programming assists individuals in becoming more independent in many life areas. Programming can occur in many different settings, including: individual instruction; group instruction and/or instruction within the natural environment. The youth transition group focuses on gaining skills for independence to move out on their own after graduation from high school. Staffing for the youth transitional services is provided at a 1:8 ratio.
Alternative School Programming: We provide a service for those students who did not make it in the school setting due to behavioral issues. The students that attend Pathway to Opportunities through the alternative school program have a strict schedule they follow that includes: academics, physical education and how to use effective coping skills and demonstrate appropriate behavior. Depending on the student, they can also work on work skills, gross motor activities and sensory stimulation. This program also has teachers from the students home district come in during the day and provide direct teaching instruction during the week. The focus of the alternative school programming is to provide the student with the appropriate coping methods and behavior adaptations to return back to the school setting. Staffing for the alternative school programming is usually at a 1:1 ratio due to the high behavior needs of the student.
In-Home Family Support (IHFS), Independent Living Skills (ILS), Semi-Independent Living Skills (SILS), Supported Living Skills (SLS): Pathway to Opportunities provides in-home services to those that are on the final stages of moving into their own independent residence or for those who are already in an independent residence but need guidance to maintain their living status. Our Pathway to Opportunities program provides assistance to individuals in their home and in the community. Areas that are worked on are life skills, money management, cleaning, busing and community involvement, social interactions and skills, cooking and medical maintenance. Staffing is based on the needs of the individual receiving services.
Outcome Based Classes: Outcome based programming is a planned, coordinated group of activities for a specific purpose. The fundamental principal is the focus on the end result. Outcome based courses are services provided with a particular end goal in mind. Prior to a student participating in an outcome based course, assessments are completed to gage where the student is at and what their learning style is. Courses are offered on a 12 week cycle but anyone can sign up at any time.
