Lifetime Opportunity Services


Adult Foster Care Services/Waiver/ICF-MR: Opportunity Matters has 17 sites currently in operation, serving individuals on the DD/CADI/TBI/ CAC/EW/AC Waivers or private pay. Our sites are licensed by the Minnesota Department of Health, and have a program or service license from the Minnesota Department of Human Services under 245B and 203 County licensure. Our professional staff members provide guidance, information and links to supportive services including transportation, personal care, financial management, and counseling.

Supported Living Services: Supported Living Services consist of a broad range of services to adults with disabilities who choose to live in a group home setting or in homes they themselves own or lease in the community. SLS may include assistance with selecting and moving into an apartment; acquiring household furnishings; common daily living activities and emergencies; becoming a participating member in community life; and, managing personal financial affairs, as well as other supports. These services help individuals exercise meaningful choice and control in their daily lives, and is designed to foster individuals' nurturing relationships, full membership in the community, and work toward their long-range personal goals.

Crisis Respite: Crisis Respite provides immediate crisis intervention for individuals experiencing a crisis. In addition to defusing crisis situations, a Behavior Management Program may be provided that can be tailored to the individuals immediate needs, along with an assessment of needs, recommendations and referrals to appropriate agencies may be made. Wrap around services may also be provided so the assessments and tools recommended can be put in place when the individual leaves crisis to ensure success in their previous or new placement. Crisis services consist of short term stays generally 30-90 days, with staffing to meet the needs of the individual. Our crisis respite units are located within Benton County and host county concurrence is needed to utilize the beds.

Two secure units - both units are located within two of our foster homes where four individuals live in the upstairs unit.

Some limitations within one unit in regards to the bathroom

Transportation can be provided if needed.

In-Home Services

In-Home Services are developed to serve individuals in their own living environment or in the community. We serve individuals with a broad range of abilities and needs. Your child or family member may require supervision for their daily living routines or assistance with accessing community resources. Your child or family member is supervised to the extent requested by the team, including assistance with managing finances, taking them out weekly for swimming, grocery shopping, community education classes, or social events, etc.

Respite Services

These services provide a break for caregivers and families and are temporary care. Opportunity Matters can provide respite services in several of our group homes for as short as one day or for as long as six months. We may be able to advance book a respite stay or have a standing repeated respite stay.
